I have been tasked to write a simple HTML survey that will be ported into Oracle and used as an email blast. But I don't know good practices in this as I've worked web pages but not email blasts.

The first file (Starting Page) will be nothing more than a logo and then a quick blurb and link to the survey. Yes, we have to have the first page.

The second page (survey) has the meat of it all. Right now there's nothing much more than placeholders as I don't know the exact verbiage that will be used.

I know that there's a few people here who know this more than me. Is this good enough or are there other things I should include/exclude?

Any help is appreciated.

Doesn't look to bad. You probably want the entire survey to be enclosed in one form tag though. also you will need a submit button for the form.

That's just it - I don't know how it'll interact with Oracle. I will put in a submit button but I don't know where it'll go at this point.
