Job application help

Ok, so I'm filling out this application online and it asks me for the first and last names of my previous supervisors.

Holy shit do I not know their last names and for some of the older ones I'm kind of fuzzy on their first ones.

I could spend like a day trying to hunt that information down, but I really want to get this done and there's no guarantee that I'll ever find out what the supervisor's last name at the target I worked at in 06 was.

Should I just guess? I've been thinking of being just totally strait with 'em and putting down "unknown" in hopes that they can either respect the honesty or don't ever read it anyway.

Don't fake it. If they call for references you'll be fucked. Call the HR department of your old job and ask.

Look on their websites and see if they show any managers for your area. Pick one of them. All HR can say is that you worked there.
 
Yup, make sure your references are in order. Also make sure you have their permission to put them down. This is common courtesy, and will allow you to find out if they are willing to say positive things about you.
 
Anyone want to be a reference? Right now I'm basing most of mine on their ability to not sound high.
