You have to coordinate with your team. Have the spy down there in the flag room or near it ready and as the heavy/medic makes their push into the ramp room/primary spawn area have the spy grab the flag and head up the stairs as the heavy cleans up with hopefully a scout nearby the bridge area to pick up the flag I'd the spy dies. Or have the spy kill engi/sap the sent in the hay room as the scout comes through the loft then wait around for enemies to go down spiral and knife them after they do. That tge engine won't be able to get the next sent high enough before the scout gets to the top of the spiral and out of the grate roomWell, I do mostly play on pubs, yes.
But even so, after a spy picks up the intel, he's got to fight his way through the entire base, either going through the straight staircase and running into the most popular sentry spot in the level, or through the spiral, fighting off every member of the opposite team, who'll know exactly where he is. And every enemy he kills is going to respawn instantly and immediately come at him again. I don't care how good a spy you are, that's a pretty tall task.

Actually, I can't speak for Mann Vs Machine. I don't understand that mode myself. If we do start a server to play that, I'm going to have to ask for help learning what to do myself. It seems a lot more complicated to me than any of the other game modes.
Left 4 Dead doesn't have any sort of upgrades to purchase, nor does it have a class system.
