He is missing an arm. The point of the book (I won't link it) is apparently that the protagonist breaks her arm and needs to be taught how to "Surf Past Fear" to regain her confidence to get back on that board.
As for whether I believe they know what they are doing, I believe they know EXACTLY what they are doing, and that this is the equivalent of those pocket watches with the naughty engraving behind the secret back so the horny Amish dude can titillate while he tills.
