The only thing I can assume is that it refers to the oracle's prediction that Xykon would visit Girard's gate first between the two remaining gates.

Roy's Prophecy (#331)

That actually covers 4 of 6 prophecies now (and the 5th is most of the way there).

1) Haley - "How can I get my speech back?" "Don't look a gift horse in the mouth." - led to date with Nale and Haley revealing her love for Elan (#393).
2) Belkar - "Do I cause the death of one of the following: Miko, Miko's stupid horse, Roy, Vaarsuvius, or you?" "Yes" - Belkar killed the oracle (#567).
3) Vaarsuvius - "How can I achieve ultimate arcane power?" "By saying the right four words to the right person at the right time for the wrong reasons." - V says "I...I must succeed" to herself to save everyone for the wrong reasons (#634).
4) Roy - "Will Xykon be within 1000 yards of Girard's gate or Kraagor's gate first?" "Of those two locations, Xykon will be within 1000 feet of Girard's gate first." - Well, he was within 1000 yards (#899).

The other two:
5) Durkon - "When will I be able to return to my dwarven homeland?" "Posthumously." - So we know Durkula will make an appearance near Kraagor's gate.
6) Elan - "Will this story have a happy ending?" "Yes, for you at least." - Rich's reassurance that this is not a tragic story.

Actually, while I don't know if Scott will ever accomplish it to the same degree, he has gone on record saying that he's got the general plot structure of Table Titans planned out for the next 2-3 years.

Pretty sure that means "I'm going to use the basic plot of this D&D game I once played"

Sure, let's assume the worst, Captain Reductive. ;)

He meant like character arcs both in-and-out of game, big twists, callbacks, foreshadowing, etc., not simply the shape of the DnD quest.

Man, Rich is really pumping out the comics lately. This is two in one week, now. Before, it was about once a week (give or take a few days).

Though I think he's ramping up to end things at #900 and take a break for a bit.

Thought #1: Huh. MitD seems to have stronger leanings towards the good side, now?
Thought #2: Why is Zykon able to float in the air? Last time he was rebuilding his body, they had to literally cart him around.
Thought #3: Interesting bit of logic they have there. It might be wrong, but it actually makes sense.
Thought #4: Front porch of Kraagar's Tomb? Uh oh.
Thought #5: OH CRAP.

Zykon might be on his way to DemiLich-dom. Once he's there, all he NEEDS is his head.

Personally I like the idea that Xykon could always fly around in half-form, and is just doing it now because there are enemies around. When there weren't, he still had the ability to do so, but didn't just to be a pain in the ass to Redcloak.
